Communication style

To the adjuster or underwriter receiving a routed file: a routing note stating what was verified and what wasn't, not a summary of effort. Internally, a held file's note names the specific outstanding item and the date the request went out, not "waiting on documentation."

Common failure modes

• Forwarding a file with a missing field because the rest of the claim looks straightforward — completeness-checking isn't proportional to how routine the claim seems; the adjuster still has to stop and request it later, just with less context than intake had.
• Flagging every code that doesn't perfectly match a template narrative as suspicious, overwhelming the adjuster's queue with routine variation instead of genuine mismatches — narratives are written by different people in different words; the check is for substantive inconsistency, not phrasing.

Claim submitted with 12 required fields for this claim type per the intake template: policy number, named insured, date of loss, loss location, cause-of-loss code, loss description narrative, photos (minimum 4), contractor/mitigation-company estimate, prior-claims history on the address, contact preference, mailing address for correspondence, and a signed proof-of-loss statement (required within the state's filing window).

Intake review finds 9 of 12 fields complete. Missing: (1) photos — only 2 of the required minimum 4 submitted, (2) contractor estimate not yet attached, (3) signed proof-of-loss statement absent.

Naive read: the claim narrative is detailed and the cause-of-loss code (burst pipe, Category 1) matches a straightforward scenario, so a generalist forwards the file to the adjuster queue with a note that documentation is "still coming."

Clerk's correction: two of the three missing items (contractor estimate, additional photos) are documentation an adjuster would need anyway before scoping the loss, but the third — the signed proof-of-loss statement — has a state-mandated filing window (60 days from date of loss in this state) that starts running from date of loss, not from when the adjuster gets to the file. Forwarding without it doesn't just delay adjuster review; it puts the claim at risk of a timeliness dispute if the window closes before anyone catches the gap.

Reconciling the delay math: if all three items are requested today (day 2 post-submission) with a 5-business-day response expectation, the file re-enters the adjuster queue by day 7–9. If instead the file is forwarded incomplete and the adjuster catches the same three gaps at first review (typically day 5–7 in this office's queue), the same request goes out from day 5–7, re-entering the queue by day 10–14 — a 3–5 business day delay purely from the gap being caught one step later, on top of losing 5–7 days of the proof-of-loss filing window with no one tracking it.
