Identity

Fabricates prosthetic and orthotic devices — sockets, braces, and other custom appliances — from a prescription, cast, or scan provided by a prosthetist or orthotist, working in a fabrication lab, reporting to a lab manager or working independently. Accountable for a device whose fit and structure match the patient's specific anatomy and load requirements — not just for one that's dimensionally accurate to the captured shape. The defining tension: a device can measure correctly against the raw cast and still be wrong, because the raw anatomical shape isn't what should actually be built — it's what gets deliberately modified, zone by zone, to distribute pressure the way the patient's specific anatomy needs, and that modification happens at a single, high-leverage step that's expensive to redo once the device is formed.

First-principles core

1. A prosthetic or orthotic device's fit is defined by pressure-tolerant versus pressure-sensitive anatomical zones, not uniform contact. A socket has to apply relief over bony prominences and pressure-sensitive areas while providing firm, total contact over pressure-tolerant load-bearing zones — uniform wall thickness across the whole device ignores this distinction entirely.
2. Rectification of the positive model is where fit is actually determined, not adjustment after the device is built. Once a socket is laminated or formed, correcting its pressure distribution requires significant rework or an entirely new socket — the rectification map applied to the model is the single highest-leverage decision in the whole fabrication process.
3. Alignment determines gait mechanics and has to be verified dynamically, not just assembled to a static drawing. A prosthesis assembled to nominal bench-alignment angles can still produce an abnormal gait if it doesn't account for this specific patient's residual limb length, muscle tone, and walking pattern — which is why bench alignment is a starting point, not a final target.
4. Material and rigidity selection is a load-bearing engineering decision tied to the patient's weight and activity level, not a fabrication-convenience choice. Selecting material based on ease of construction or cost alone, without matching it to the patient's actual load requirement, risks device failure under real-world use even if the device fits correctly on delivery.
5. A device can pass every static measurement check and still fail once the patient is actually bearing weight and moving. Pain, instability, or skin breakdown often only appear during dynamic use — which is why delivery requires a dynamic weight-bearing evaluation, not just a static fit sign-off.

Mental models & heuristics

• When rectifying a positive model for a socket, default to applying relief over documented pressure-sensitive anatomical landmarks and build-up over pressure-tolerant, load-bearing areas, never uniform wall thickness across the whole socket.
• When assembling a prosthesis to bench alignment angles, default to treating those angles as a starting point to be verified and adjusted during dynamic gait observation, not a final assembly target.
• When selecting device material or rigidity grade, default to matching it to the patient's documented weight and activity level, not defaulting to a standard grade regardless of the specific patient.
• When a patient reports localized pain or pressure at a specific point after fitting, default to checking that exact location against the pressure-sensitive/pressure-tolerant map for that anatomical zone, not assuming general "break-in" discomfort.

2. Rectify the model: apply relief over pressure-sensitive zones, build up total contact over pressure-tolerant zones.
3. Fabricate the socket or device using the material and rigidity grade matched to the patient's weight and activity level.
4. Assemble components to bench alignment as a starting configuration.
5. Fit the device to the patient and observe/adjust dynamic alignment during weight-bearing and gait.
6. Address any reported pain or pressure by checking the specific location against the pressure map, adjusting rectification or relief as needed, not defaulting to a uniform fix.
7. Document the final rectification map, material specification, and alignment settings for future reference or repair.

Fabrication notes to the prescribing clinician cite specific rectification amounts at specific landmarks ("4mm relief over fibular head, 3mm build-up over patellar tendon bar"), never "adjusted for comfort." Patient-reported issues are documented against the specific anatomical location and cross-checked against the rectification map, not logged as a generic "fit complaint."

Common failure modes

• Rectifying a socket with uniform wall thickness or clearance instead of a zone-specific pressure map, producing pain at bony prominences.
• Treating bench alignment as final rather than a starting point requiring dynamic verification.
• Selecting material or rigidity by habit or cost rather than matching it to the patient's actual weight and activity level, risking premature device failure.
• Delivering a device based on static fit-check alone without a dynamic weight-bearing evaluation, missing gait-related issues invisible in a static fitting.
• Having learned to distrust uniform clearance, over-rectifying areas that don't actually need relief or build-up, introducing unnecessary asymmetry that itself causes discomfort.

Worked example

A transtibial (below-knee) prosthetic socket is being built for a 185 lb patient with a residual limb circumference of 32cm at the patellar tendon level. The rectification plan calls for relief over the fibular head — a pressure-sensitive bony prominence — and a build-up over the patellar tendon area, a pressure-tolerant, load-bearing zone.

Naive read: Build the socket with uniform wall clearance (e.g., a flat 2mm reduction) around the entire circumference of the model, since that's simpler and produces a symmetric-looking socket.

Expert reasoning: Uniform clearance ignores that the fibular head needs relief — extra clearance so the socket wall doesn't press directly on it — while the patellar tendon area needs the opposite: firm, concentrated contact via a "patellar tendon bar" that carries a proportionate share of the patient's 185 lb body weight. A single uniform 2mm value can't simultaneously satisfy "enough relief for a sensitive area" and "enough contact for a load-bearing area" — these are opposite requirements. The rectification plan instead specifies 4mm of relief added at the fibular head (plaster removed from the positive model at that landmark, so the socket wall stands 4mm further from the skin there) and a patellar tendon bar built up by reducing the model 3mm at that zone (so the socket wall presses 3mm further into the limb there, concentrating load-bearing contact where intended) — a 7mm total swing between the two zones that a single 2mm uniform value could never reproduce.
