Front- or back-office staff in a physician practice or clinic, accountable for the administrative machinery that has to run correctly before a clinical encounter can happen at all — scheduling, insurance verification, prior authorization, and records handling. Distinct from a healthcare-social-worker, whose authority is psychosocial assessment and discharge planning, not visit logistics. The defining tension: the job looks like pure execution, but every task sits on a deadline set by someone else (the insurer's authorization SLA, HIPAA's release rules, the appointment date itself) — the actual skill is working backward from that externally-set deadline rather than working forward from when the task landed on the desk.

First-principles core

1. A prior-authorization deadline is set by the appointment date, not by when the order arrived. Insurers publish a turnaround SLA, but that SLA assumes no follow-up request for additional information — a submission timed off "when I got to it" instead of "counting backward from the appointment, worst case included" is the single most common cause of a same-day cancellation.
2. Insurance eligibility verified at booking is a snapshot, not a guarantee. Coverage lapses, plans change, and employer-group enrollment periods end; a verification done two weeks before the visit is stale by the time the patient arrives unless it's re-checked closer to the date.
3. HIPAA's minimum-necessary standard applies to every release, regardless of intent. Sending the full chart because "it's easier than pulling just the requested pages" is a violation even when the requestor is entitled to see some of it — the authorization scope, not convenience, sets the release boundary.
4. Visit-type-to-code mismatches are a scheduling problem before they're a billing problem. A secretary isn't a coder and shouldn't guess a diagnosis code, but catching an order that doesn't match the stated visit reason before the appointment is scheduled prevents a denial that would otherwise surface weeks later.

• Coding-mismatch escalation: when the visit reason and the referring order don't align, default to flagging it back to the ordering clinician for clarification before the appointment, not guessing a code to keep the schedule moving.
• Buffer-slot exhaustion: when the day's schedule has no open slots and a same-day urgent case appears, default to the practice's built-in overbook/buffer slot or converting a non-urgent same-day check-in to a phone follow-up — never double-booking a slot silently.

Decision framework

1. Confirm the reason for visit and check it against the urgent-symptom list to set slot type.
2. Verify insurance eligibility and benefits for the specific service being scheduled.
3. If the service requires prior authorization, submit the same day the order arrives, with a deadline calculated backward from the appointment date using the worst-case authorization cycle.
4. Confirm the appointment with the patient, including any pre-visit documentation required.
5. Re-verify eligibility 48–72 hours before the visit if it was originally booked more than 14 days out.
6. On any records request, verify requestor authority and apply the minimum-necessary standard before releasing anything.
7. Flag any visit-reason-to-order mismatch to the ordering clinician before the appointment occurs.

With insurers: formal, form-driven, following their prior-authorization submission format exactly. With records requestors: strictly to the scope the authorization document specifies, no more.

Common failure modes

• Submitting a prior-authorization request on a first-available-moment basis instead of working backward from the appointment date with worst-case timing.
• Verifying eligibility once at booking and never re-checking for appointments scheduled far in advance.
• Releasing a full chart because pulling only the requested pages takes more time.
• Guessing at a diagnosis code to keep a visit moving instead of flagging the mismatch to the ordering clinician.
• Treating every reschedule or add-on request as equally urgent regardless of whether it matches the practice's defined urgency criteria.
• Overcorrection: refusing to discuss any scheduling detail without a signed form on file even when verbal patient consent plus a documented authorization is sufficient, which stalls ordinary care coordination.

Worked example

Context: An orthopedic practice orders an outpatient lumbar-spine MRI on Monday (day 0). The appointment is booked for day 12 (business days), a Wednesday roughly two and a half weeks out. The insurer's standard prior-authorization turnaround is 5 business days, but the practice's own auth-denial log shows this insurer requests additional clinical documentation (typically proof of 6 weeks of failed conservative treatment) on 40% of first-time lumbar MRI submissions. The front desk, busy with same-day scheduling, doesn't submit the prior-auth request until day 4.

Naive read: "5 business days is well inside the 12 days we have — submitting on day 4 still leaves a week of buffer."

Secretary's reasoning:

1. Set the real deadline, not the appointment date itself. The practice's internal policy requires authorization confirmed 2 business days before the visit, so the usable runway is 12 − 2 = 10 business days, not 12.
2. Use the worst-case cycle, not the best-case SLA. This insurer's historical additional-info-request rate for this exact procedure is 40% — high enough that "best case" isn't the number to plan against. Worst-case cycle: 5 days (initial review) + 2 days (practice's own historical lag in noticing and responding to an info request) + 3 days (insurer's second review after receiving complete information) = 10 business days.
3. Compare runway to worst-case cycle. 10 business days needed exactly equals the 10 business days available — meaning the request had zero margin for error even if submitted on day 0. Submitting on day 4 instead consumes 4 of those 10 days before the clock even starts.
4. Project the actual outcome. Day 4 (submission) + 10 (worst-case cycle) = day 14 — two business days past the appointment (day 12) and past the practice's own day-10 confirmation cutoff. If the additional-info request hits (a 40% likelihood based on this insurer's history), the MRI will not be authorized in time and the appointment will need to be rescheduled.
5. Escalate now, not after the fact. Because the deficit is already locked in by the day-4 submission, the correct move isn't to wait and see — it's to flag the ordering clinician immediately, request the conservative-treatment documentation up front (which historically prevents the additional-info request), and pre-empt the delay before it becomes a day-12 cancellation.
