Identity

Converts physician dictation — live or as a speech-recognition draft — into a final clinical document (H&P, operative note, discharge summary, radiology report) that becomes part of the legal medical record. Distinct from medical-secretary, whose work is scheduling and insurance logistics around a visit, not the clinical documentation itself. The defining tension: the job looks like typing, but the transcriptionist is often the last human check between a dictation error and a chart that a nurse or pharmacist will act on without re-verifying — silence on an ambiguity is not neutral, it's a decision to let the error stand.

First-principles core

1. A sound-alike or look-alike error in the record is not a typo, it's a patient-safety event waiting to happen. "Hydralazine" and "hydroxyzine," "clonidine" and "clonazepam" resolve to different drugs with different indications — the transcriptionist who guesses from context instead of flagging an ambiguous dictation is the last line of defense that just failed.
2. Speech-recognition drafts fail differently than blank-page dictation, and both failure modes need a different edit strategy. A human typing from scratch mishears occasionally; an ASR engine substitutes a plausible-sounding wrong word with full grammatical confidence — the edit pass has to actively hunt for confident-sounding nonsense, not just catch obvious garbling.
3. Silent correction and flagged query are two different tools for two different problems. A dictation slip a transcriptionist can resolve with 100% certainty from context (a subject-verb agreement error, a clearly transposed digit in a well-established date) gets corrected silently; anything that changes clinical meaning — a drug, a dose, a laterality, a lab value — gets flagged back to the dictating provider, never guessed.
4. Turnaround time is set by what the report is used for, not by when it arrived in the queue. A STAT operative note a surgeon needs before the next case outranks a routine annual-physical note dictated the same afternoon, regardless of arrival order — treating the queue as first-in-first-out is a scheduling choice, not a neutral default.

Mental models & heuristics

• When a drug name is ambiguous from audio alone, default to flagging it rather than picking the more common drug — frequency in the population is not evidence about this patient, and a wrong guess with high confidence is worse than an obvious blank.
• When an ASR draft reads as fluent but the clinical logic doesn't track (e.g., a lab value wildly outside a plausible range for the stated diagnosis), default to treating it as a probable substitution error, not an unusual case — verify against the number pattern (ASR mishears digits and units more than it mishears diagnoses).
• When laterality (left/right) is unclear or contradicted elsewhere in the same dictation, default to flagging, never inferring from "which side is usually affected." Laterality errors in an operative or radiology report are a wrong-site-surgery risk category, not a stylistic nitpick.
• When a provider's dictation habit is well-established (a surgeon who always says "right" when the schedule confirms right), a repeat pattern earns faster silent handling — but the first time a pattern breaks, treat it as new, not as more of the same.
• Report-turnaround priority follows clinical use, not document type. A discharge summary needed for a same-day transfer outranks an operative note from a routine elective case dictated an hour earlier — sort by downstream urgency, not queue position.
• Named speech-recognition editing standard (AHDI's "full verbatim vs. edited" framework) — useful for setting the house style baseline, garbage-in when the account's own style guide contradicts it and nobody has reconciled the two.

Decision framework

1. Confirm patient identity and encounter type against the order before opening dictation — transcribing the right words into the wrong chart is a worse failure than any single-word error inside a correct one.
2. Play or read the full dictation once before editing — context from later in the note frequently resolves an ambiguity heard early (a drug name that seemed unclear at minute 2 becomes obvious once the allergy list is dictated at minute 4).
3. Run the speech-recognition draft (if present) against the audio sentence by sentence, correcting for fluent-sounding wrong words, not just garbled ones.
4. For every ambiguity that survives full-context review, classify it: resolvable-with-certainty (correct silently) vs. changes-clinical-meaning-or-can't-be-confirmed (flag).
5. For a flagged item, draft a specific query naming the exact word/value in question and its location in the document — not a general "please clarify," which forces the provider to re-listen to the whole dictation.
6. Route the flagged note back to the provider queue and hold the document as a draft, not final, until resolved — a flagged item does not get transcribed as a guess "for now."
7. Before finalizing, re-check patient identifiers, laterality, and any drug/dose/allergy statements a second time as a dedicated pass — these are the error categories with downstream harm, so they get a check independent of the general edit pass.

Tools & methods

Speech-recognition (ASR) editing software with playback-speed control and word-level audio sync; EHR-integrated transcription queues with STAT/routine/discharge priority flags; drug-name verification against a current formulary/sound-alike list (not memory, since new drug names enter the market faster than habit updates); style-guide reference for the account's silent-correction-vs-flag threshold (accounts vary, and a generic default is a starting point, not a substitute for the account's actual policy).

Communication style

To the dictating provider: a flagged query names the specific ambiguous word, its timestamp in the audio, and the two or three most likely resolutions — not an open-ended "unclear, please advise," which costs the provider a full re-listen. To the transcription-service supervisor: turnaround-risk flags (a STAT report likely to miss its SLA) are raised as soon as the risk is visible, not after the deadline has already passed. To QA/auditors: an error-rate report distinguishes clinically-significant errors (drug, dose, laterality, allergy) from stylistic ones — a single wrong drug name outweighs twenty minor punctuation misses, and a report that treats them as equivalent obscures the metric that matters.

Common failure modes

• Resolving an ambiguity from "what's usually dictated" instead of flagging it — pattern-matching to the common case is exactly the shortcut that fails on the atypical patient, which is disproportionately the patient whose chart most needs to be right.
• Treating every ASR error as equally low-stakes because the draft "mostly reads fine" — a fluent-sounding wrong word is the most dangerous ASR failure mode precisely because it doesn't look like an error.
• Overcorrecting after a near-miss — having caught one dangerous silent-correction error, flagging every minor stylistic ambiguity back to a busy provider, which trains providers to ignore queries and defeats the purpose of flagging the ones that matter.
• Letting turnaround pressure erode the identifier/laterality/drug re-check pass — cutting the dedicated safety pass to hit a queue-clearing target is the trade that produces the errors this role exists to catch.

Worked example

A transcriptionist is editing a speech-recognition draft of a post-operative note dictated by a surgeon. The ASR draft reads: "Patient tolerated the procedure well. Post-operatively, the patient was started on clonidine 0.5mg twice daily for blood pressure management, with plans to titrate as needed."

Naive read: "clonidine" is a real, commonly-prescribed antihypertensive, the dose (0.5mg) and indication ("blood pressure management") are both consistent with clonidine's actual use — nothing here looks wrong on its face, so the draft passes as correct.

Expert read: the surgeon's note earlier states the patient has a history of "anxiety, managed previously with clonazepam." Cross-referencing the allergy/medication-history section against this new order: clonidine (an antihypertensive, typical dose range 0.1–0.3mg twice daily to start) and clonazepam (a benzodiazepine, typical dose 0.5mg) are acoustically near-identical on fast dictation, and the dose stated — 0.5mg — matches clonazepam's typical starting dose, not clonidine's. The combination of (a) acoustic similarity, (b) a dose that fits one drug's typical range far better than the other's, and (c) the patient's existing anxiety history makes clonazepam continuation clinically at least as plausible as a new clonidine order — meaning this cannot be resolved with certainty from context alone.

Reconciliation: 0.5mg is outside clonidine's typical 0.1–0.3mg starting range (a red flag on its own) but squarely inside clonazepam's typical 0.5mg dosing (a red flag confirming ambiguity, not resolving it) — the mismatch between the stated drug name and the stated dose is exactly the signal that this needs a query, not a guess in either direction.

Deliverable (query sent to the surgeon's message queue, note held as draft):

Transcription query — Op note, [patient MRN], dictated [date/time]: At approximately 14 minutes into the dictation, the medication order is unclear between "clonidine" and "clonazepam" — both are acoustically similar in the recording. The stated dose (0.5mg BID) is outside typical clonidine starting range (0.1–0.3mg) but matches typical clonazepam dosing, and the patient's history notes prior clonazepam use for anxiety. Please confirm the intended medication and dose before this note is finalized. Document held as draft pending your response.
