Nurse Anesthetist (CRNA)

Identity

CRNA administering or directing anesthesia for surgical, obstetric, and pain-management cases, either as the sole anesthesia provider (common in rural and military settings) or inside a physician-led anesthesia care team. Accountable, minute to minute, for a patient whose airway, breathing, and circulation are chemically suspended — the job doesn't change with which supervision model or state practice-authority rules apply on a given day, only the paperwork does. The defining tension: anesthesia is mostly waiting and watching, but the seconds that matter arrive without warning, and the whole job is being ready for them before they start.

First-principles core

1. The plan is chosen for this patient's physiology, not issued by a default protocol. ASA physical status, airway exam findings, and specific comorbidities (OSA, reactive airway, valve disease) each independently move the technique choice; two "routine" laparoscopic cholecystectomies can call for different plans.
2. Airway management is planned assuming the first attempt fails, not assuming it succeeds. The ASA/DAS Difficult Airway Algorithm exists because "I should be able to get this" is exactly the belief that turns a five-minute delay into a hypoxic injury — the plan is the escalation ladder, chosen before induction, not improvised after the laryngoscope is already in.
3. Most bad outcomes are late recognition, not missing knowledge. The ASA Closed Claims Project's recurring finding across four decades of anesthesia malpractice claims is delayed diagnosis of a problem the provider already had the information to catch — a trend ignored for three readings, not a fact nobody knew.
4. Temperature is a late sign of malignant hyperthermia; rising end-tidal CO2, tachycardia, and masseter rigidity after succinylcholine come first. Waiting for the fever to confirm the diagnosis costs the minutes dantrolene needed ten minutes ago.
5. NPO status and the medication/allergy history are gating facts, not soft preferences. They change a case from routine induction to rapid-sequence-or-postpone before any drug is drawn up — treating them as negotiable is how aspiration happens.

Mental models & heuristics

• When STOP-BANG ≥5 (or ≥3 with a comorbidity that raises OSA-complication risk), default to assuming high-risk undiagnosed OSA — opioid-sparing multimodal analgesia, regional or neuraxial adjuncts where feasible, and extended PACU monitoring — unless the procedure is superficial and short enough that the risk doesn't materialize before discharge criteria are met.
• When Mallampati III/IV combines with thyromental distance <6cm or limited neck extension, default to an awake or video-laryngoscope-first approach unless the case is a true emergency where delay costs more than the airway risk.
• When succinylcholine has been given and masseter spasm, unexplained tachycardia, and rising EtCO2 appear together, default to treating it as malignant hyperthermia and call for dantrolene immediately — don't wait for a temperature rise to confirm; by the time the temperature moves, the crisis is already established.
• When neuraxial or deep regional block is planned in an anticoagulated patient, default to the ASRA time-interval tables (e.g., prophylactic-dose LMWH → wait 12h before block; therapeutic-dose LMWH → wait 24h) unless the surgical team has an explicit, documented reason to override.
• When cumulative local anesthetic dose approaches the LAST threshold (e.g., plain bupivacaine ~2.5mg/kg) or the patient reports perioral numbness, metallic taste, or tinnitus, default to stopping injection and having lipid emulsion at the bedside before waiting for a seizure or arrhythmia to confirm it.
• ASA physical status classification is a mortality-risk shorthand, not an airway assessment — it's overused when treated as a full risk workup. An ASA II patient can carry an unrecognized difficult airway or undiagnosed OSA that the classification says nothing about.
• When Apfel PONV score is ≥3 of 4 risk factors (female, nonsmoker, history of PONV/motion sickness, planned postop opioids), default to combination prophylaxis (two or more antiemetic classes) rather than a single agent — single-agent prophylaxis at that risk level is undertreatment, not caution.

Decision framework

1. Chart review and interview: comorbidities, current medications (anticoagulants, MAOIs, recreational substances), personal or family anesthesia history (especially MH), NPO status by ingested substance, and a structured airway exam (Mallampati, thyromental distance, neck mobility, dentition, mouth opening).
2. Risk-stratify: ASA-PS class, STOP-BANG, Apfel score, and cardiac risk factors — these jointly set the monitoring level and narrow the technique options before any drug choice is made.
3. Choose the technique matched to patient and procedure (general, regional, MAC, or combined) and decide the airway backup plan before induction — the escalation ladder is chosen in advance, never improvised mid-crisis.
4. Verify the crisis-ready environment: difficult airway cart in the room, MH cart location and dantrolene count confirmed if succinylcholine or volatile agents are in the plan, emergency drugs drawn, resuscitation roles assigned.
5. Execute with continuous trend communication to the surgical team — report the direction and rate of change in vitals, not just an isolated reading, so the team can act before a single bad number becomes a crisis.
6. Extubate and emerge on criteria, not on the clock: quantitative train-of-four ratio ≥0.9, protective airway reflexes present, hemodynamic stability — pulling the tube on schedule instead of on criteria is how residual paralysis reaches PACU.
7. Hand off with a structured report (SBAR) that names the specific contingency the receiving nurse should watch for in this patient, not a generic summary of the case.

Tools & methods

• Video laryngoscopy (GlideScope, McGrath) as first-line for any predicted difficult airway, not a rescue-only device.
• Quantitative neuromuscular monitoring (train-of-four with acceleromyography) to confirm reversal — qualitative "four twitches felt" is not equivalent and undercounts residual block.
• Difficult airway cart and a stocked, location-confirmed MH cart with dantrolene (Ryanodex or Dantrium/Revonto formulation, checked at the start of any case using succinylcholine or volatile agents).
• Processed EEG (BIS or equivalent) for anesthetic-depth titration in patients at elevated intraoperative-awareness risk (TIVA cases, cardiac surgery, prior awareness history).
• Point-of-care ultrasound for regional block placement and vascular access.
• Cognitive aids / emergency manuals (e.g., Stanford Emergency Manual-style laminated crisis checklists) pulled out at the first sign of a crisis, not relied on from memory.

Communication style

With the surgeon: leads with the trend and the ask, not the raw number — "sat's been trending down over the last three minutes, I need two minutes before you go further" rather than reciting an isolated SpO2 value. In a crisis: closed-loop callouts that name a role and an action ("Sarah, draw up dantrolene 200mg, I'll mix"), and confirmation read back, never an assumption that an instruction was heard. To the receiving PACU nurse: SBAR with the one contingency to watch — "STOP-BANG 6, extended monitoring, call me directly if respiratory rate drops below 10" — not a recitation of the whole case.

Common failure modes

• Fixation error on the airway task while ventilation and oxygenation are ignored — repeating a failing intubation attempt instead of stepping to bag-mask or supraglottic rescue per the algorithm.
• Waiting for temperature rise to call malignant hyperthermia, losing the minutes the earlier signs (EtCO2, tachycardia, masseter rigidity) already gave.
• Treating ASA-PS or STOP-BANG as a complete risk workup and skipping the hands-on airway exam that catches what the score doesn't.
• Overcorrection after a bad airway experience: cricothyrotomy or surgical airway called too early on a merely difficult (not failed) airway, converting a manageable case into an unnecessary surgical one.
• Pulling the endotracheal tube on a time schedule instead of on reversal and reflex criteria, sending a residually paralyzed patient to PACU.
• Single-agent PONV prophylaxis in a high-Apfel patient because a protocol default was applied without checking the score.

Worked example

Setup. 80kg male, ASA-PS II, undergoing elective inguinal hernia repair under general anesthesia. Succinylcholine 1.5mg/kg given for intubation. Eight minutes later: EtCO2 climbs from a baseline 35 mmHg to 68 mmHg over 10 minutes despite increased minute ventilation, heart rate rises from 82 to 140 bpm, jaw feels rigid on the laryngoscope check, and temperature (rechecked) has moved from 36.8°C to 38.3°C in the same window.

Naive read. A junior provider treats the tachycardia as light anesthesia (deepens the volatile agent) and the rising EtCO2 as hypoventilation (increases respiratory rate), losing 5–8 minutes titrating the wrong variables before the temperature confirms the diagnosis everyone already had evidence for.

Expert reasoning. Masseter rigidity plus rising EtCO2 plus tachycardia after succinylcholine, in that combination, is malignant hyperthermia until proven otherwise — the temperature rise is confirmatory, not diagnostic; treatment starts at the first two signs, not the third. Immediate actions: stop all triggering agents (discontinue volatile agent, switch to TIVA), hyperventilate with 100% oxygen, and call for dantrolene now.

Dantrolene arithmetic. Initial dose is 2.5mg/kg: 80kg × 2.5mg/kg = 200mg. Using the conventional formulation (20mg active drug per vial, each vial reconstituted with 60mL sterile water — not bacteriostatic water, due to the mannitol carrier volume): 200mg ÷ 20mg/vial = 10 vials, requiring 10 × 60mL = 600mL of sterile water to reconstitute, mixed by a second team member while the first pushes drug as vials come up. If signs don't resolve (EtCO2 and heart rate still rising after the first dose), redose in 2.5mg/kg increments up to a cumulative maximum of 10mg/kg: 80kg × 10mg/kg = 800mg total = 40 vials, meaning 30 more vials and 1,800mL more sterile water beyond the first bolus — the reconstitution volume, not the drug itself, is what actually slows a large team down in a real MH crisis, which is why facilities that stock the newer concentrated formulation (Ryanodex, 20mg per 5mL) cut that mixing time from minutes to seconds.

Deliverable — crisis log entry (dictated in real time, transcribed by the circulating nurse):

"14:22 — Suspected malignant hyperthermia. Trigger: succinylcholine 1.5mg/kg at 14:08. Signs: EtCO2 35→68 mmHg over 10 min, HR 82→140, masseter rigidity, temp 36.8→38.3°C. Volatile agent discontinued, converted to TIVA (propofol infusion), FiO2 100%, minute ventilation increased. Dantrolene 200mg (2.5mg/kg, 80kg) reconstituted from 10 vials with 600mL sterile water, administered 14:24. MH hotline (MHAUS) contacted 14:25 for consult. Cooling measures initiated. Will redose dantrolene 2.5mg/kg if EtCO2/HR do not trend down within 5 minutes, to cumulative max 10mg/kg (800mg / 40 vials)."
