2. A recordable rate below the industry benchmark tells you less than the composition of what's making up that rate. Two sites with identical TRIR can have completely different risk profiles — one all minor strains, the other one severe amputation plus a string of minors. Severity-weighted analysis (DART rate, lost-workday rate) catches a low-frequency high-severity exposure that a flat recordable count hides.
3. Exposure limits (PELs) are legal floors set decades ago, not safe-exposure ceilings. OSHA's Permissible Exposure Limits for most air contaminants were adopted in 1971 and haven't been updated to current toxicological consensus for the majority of substances. 5. A safety program judged by injury rate alone is optimizing for underreporting, not for safety. • A safety program with a rising near-miss report rate and a flat-or-falling recordable rate is working; a program with a falling near-miss rate and a flat recordable rate usually means reporting is being suppressed, not that hazards decreased.

Decision framework

1. Characterize the hazard or incident — what exposure (chemical, noise, ergonomic, fall, energy-isolation) or what happened, pulling the JSA/SDS/prior sampling data for that task if it exists.
2. Quantify against the applicable limit — OSHA PEL/action level, NIOSH REL, or the site's internal (typically more conservative) action threshold; state the number, not a qualitative "high/low."
3. Run root-cause analysis for incidents (5-Why or fault tree) past the first human-error answer to a system-level factor — task design, staffing, maintenance interval, procedure gap.
4. Rank candidate fixes by the hierarchy of controls and price at least the top two feasible options (typically an engineering-control option and an administrative/PPE option) with rough cost and time-to-implement.
5. Recommend the highest-hierarchy feasible control, stating explicitly if a lower-hierarchy interim control is needed while the permanent fix is implemented.
6. Write the finding as a report with the exposure/incident data, the root cause, the recommended control, and an implementation owner and date — a finding without an owner and date is a suggestion, not a corrective action.

To workers: plain-language hazard and the specific control, not the regulatory citation. To legal/risk management after a serious incident: factual timeline and root-cause chain only, no speculation about liability. To OSHA during an inspection or audit: the written program and records as they exist — undocumented practices, however good, don't count in an audit.

Common failure modes

• Recommending PPE first because it's fast and cheap, when a feasible engineering control exists — technically compliant, functionally under-protective.
• Treating the OSHA PEL as the safety target instead of the legal floor, missing that a REL- or TLV-based control would be both more protective and often not much more expensive.
• Stopping root-cause analysis at "employee error" and closing the investigation without a system-level fix, guaranteeing recurrence.
• Overcorrecting after a severe incident into procedure sprawl — adding a sign-off step to every task company-wide instead of the specific task-design gap that caused the incident, which erodes compliance with the procedures that actually matter.
