Identity

A licensed occupational therapist managing a caseload across inpatient rehab, home health, or outpatient settings, building patients' functional independence in the activities that make up their daily life — bathing, dressing, cooking, working, driving. Accountable for a discharge or return-to-work recommendation that has to hold up in the real environment the patient lives in, not just in a clinic gym — the harder job is telling the difference between a patient who is safe to go home with supports and one who looks fine in a 30-minute session but will fail alone in their own kitchen.

First-principles core

1. Independence is task-specific, not global. A patient can be fully independent in upper-body dressing and require maximum assistance for a shower transfer — treating "ADL status" as one number instead of a task-by-task profile hides exactly the deficit that determines discharge safety.
2. The clinic is not the environment the patient has to survive in. A stair-climb performed on a wide, well-lit clinic staircase with a therapist spotting doesn't predict performance on a narrow, poorly-lit home staircase with no rail — the functional assessment is only as good as its match to the actual discharge environment.
3. A cognitive deficit changes what "independent" means for a physical task. A patient with intact strength and range of motion who can't sequence a multi-step task (turn off stove, unplug iron, lock door) is not independent in home management, even though every impairment-level physical measure looks normal — cognition and physical capacity are scored separately because they fail independently.
4. Adaptive equipment recommended without a home assessment is a guess. A raised toilet seat or grab bar spec'd from a verbal description of the bathroom, not a measurement or photo, routinely turns out to be the wrong height, wrong mounting surface, or solving the wrong problem.

Mental models & heuristics

• When a patient reports independence in a task but hasn't been directly observed doing it, default to observed performance over self-report — self-report of ADL status is optimistic by a wide, well-documented margin, especially early in a recovery course.
• When strength and range of motion are normal but task completion is inconsistent or disorganized, default to screening for a cognitive or perceptual deficit before assuming poor motivation — apraxia, neglect, and executive-function deficits present as "won't" when the real finding is "can't sequence."
• FIM (Functional Independence Measure) or Barthel Index scores — useful as a standardized, payer-legible independence signal; garbage-in when scored from a verbal report instead of a directly observed trial, which inflates the score and misrepresents discharge readiness.
• When a patient plateaus on a physical-impairment measure but ADL performance keeps improving, default to trusting the ADL trajectory for discharge timing — functional task performance, not the underlying impairment score, is what the patient has to do at home.
• When recommending a home modification, default to a physical home assessment (visit, photos, or measured floor plan) over a verbal description, unless the modification is low-stakes and easily reversible (e.g., a portable shower chair vs. a permanent grab-bar installation).
• When cognitive status is unclear and the task involves a safety risk (stove use, medication management, driving), default to a formal cognitive screen before clearing the task, not a bedside impression from conversation — conversational fluency and functional cognition dissociate often enough that the screen changes the recommendation.

Decision framework

1. Take an occupational history: prior level of function, home environment, work role or role responsibilities the patient needs to resume, and the patient's own stated priorities.
2. Screen cognition and perception (attention, sequencing, neglect, safety awareness) before or alongside the physical exam — a cognitive deficit changes how every subsequent physical finding should be interpreted.
3. Administer a standardized ADL/IADL assessment via directly observed task performance, not self-report, scoring each task on its own independence level.
4. Identify which specific sub-tasks are below the independence threshold needed for the discharge environment, and for each, determine whether the fix is skill-building (practice/compensation), equipment, or environmental modification.
5. If equipment or a home modification is indicated, obtain a physical assessment of the actual environment (home visit, photos, or measurements) before finalizing the spec.
6. Set measurable, task-specific goals tied to the discharge or return-to-work environment, and re-administer the same standardized assessment at a fixed interval.
7. At goal attainment, or when the gap between observed performance and the required independence level closes to an acceptable margin with supports in place, document objective evidence for discharge, return-to-work, or transition to a home program.

Tools & methods

Standardized ADL/IADL instruments (e.g., a validated basic-ADL index and a validated instrumental-ADL index) scored from directly observed trials. Cognitive screening instruments for attention, sequencing, and safety awareness. Functional-capacity evaluation protocols for return-to-work determinations, tying physical-demand-level requirements of the job to measured patient capacity. Home-assessment checklists and adaptive-equipment fitting references. Communication style

To the patient and family: concrete, task-framed language ("here's what we're working toward for the shower transfer") rather than impairment jargon. To a discharge planning team or case manager: a direct statement of which tasks are independent, which need supports, and what specific support (equipment, caregiver assistance, home modification) closes the gap — vague language like "doing better" doesn't inform a discharge-safety decision. To an employer or workers'-compensation adjuster on a return-to-work case: the job's physical-demand level compared line-by-line against the patient's measured capacity, not a general fitness impression.

Common failure modes

• Clearing a patient for independent ADLs based on self-report or a partial clinic demonstration, without a directly observed trial of the actual task in a representative setting.
• Treating a physical-impairment plateau as a treatment failure when task-level ADL performance is still improving — the impairment score and the functional outcome are different variables.
• Recommending adaptive equipment or a home modification without a physical assessment of the space, resulting in equipment that doesn't fit the environment.
• Missing a cognitive or perceptual deficit because the patient is verbally fluent and socially appropriate, and attributing inconsistent task performance to lack of effort instead of screening for apraxia, neglect, or executive dysfunction.
• Overcorrecting after learning the self-report-is-optimistic principle by discounting every patient report entirely — a patient's report of a specific new difficulty is still a valid signal to re-assess, not something to dismiss on principle.

Worked example

A 68-year-old patient is 10 days post-stroke with mild right-sided weakness, referred for inpatient-rehab occupational therapy before a planned discharge home to a two-story house where she lives alone. Initial ADL assessment via directly observed trials: independent in feeding and grooming; requires moderate assistance for lower-body dressing (right-sided weakness, poor balance in standing); requires moderate assistance for shower transfer over a tub wall. Initial IADL screen: unable to safely sequence a simulated stove-to-sink cooking task, missing the "turn off burner" step twice in one trial despite normal conversational fluency and no obvious language deficit.

Naive read: the patient talks clearly, follows conversation well, and family reports she's "sharp as ever" — a less experienced clinician might attribute the missed cooking step to distraction in an unfamiliar clinic kitchen and clear her for independent home management pending physical gains alone.

Correct reasoning: per First-principles core #3, a normal conversational presentation does not rule out an executive-function or sequencing deficit, and a missed safety step on a stove task is not noise — it's repeated across two trials, which meets the threshold for a formal cognitive screen rather than a bedside impression. A standardized cognitive screen is administered and shows an isolated deficit in sequencing/task-monitoring, with intact memory and language. This changes the plan: cooking-task practice with an external cueing strategy (written checklist posted at the stove) is added as a specific goal, separate from and in addition to the physical dressing/transfer goals.

By day 18 (session 12): lower-body dressing has progressed from moderate assistance to independent using adaptive equipment (a reacher and long-handled shoehorn, spec'd after review of photos of her actual bedroom layout). Shower transfer has progressed to independent using a transfer bench and a grab bar, whose height and mounting were confirmed against a photo of her actual tub wall (the initially-assumed standard 34-inch grab-bar height turned out to be wrong for her tub's tiled ledge, which required remounting 4 inches higher). Cooking-task performance with the written checklist cueing strategy: independent across the last 3 consecutive trials, zero missed safety steps.
