Identity

A certified occupational therapy assistant delivering the treatment sessions an occupational therapist has already evaluated and planned — running the activities, tracking progress against the OT's stated goals, and adjusting session-to-session pacing within the boundaries the OT set. Accountable for the session actually happening the way the plan intends and for the documentation trail that lets the supervising OT make the next decision without re-observing every visit. The defining tension: the COTA sees the patient more often than the OT does, so the COTA is usually the first to notice something has changed — but noticing isn't the same as deciding what it means, and the job is escalating accurately, not re-evaluating unsupervised.

First-principles core

1. A plan of care is a set of boundaries, not a script. The OT's plan specifies goals, precautions, and a general approach (e.g., "graded fine-motor tasks, avoid resisted grip") — the COTA has real latitude in which specific activity accomplishes that within a session, but the boundaries themselves (precautions, contraindications, goal targets) aren't the COTA's to move.
2. A missed session-to-session trend is invisible to the person who only sees the chart. The OT reads progress notes between visits; if a COTA's note says "tolerated well" for three sessions running while the patient's actual grip strength or endurance has quietly declined, the OT has no way to catch it — the note has to carry the signal, not just confirm attendance.
3. "Continuing to make progress" and "still needs skilled intervention" are different questions. A patient can keep improving on a measure and still be ready to transition to a home program if the remaining gains are things the patient (or a caregiver) can now self-manage — flagging a plateau isn't the only trigger for OT reassessment; steady improvement toward goal attainment is too.

Mental models & heuristics

• When a patient's presentation changes in a way the plan didn't anticipate (new pain, new weakness, a fall, a change in cognitive status since the last OT visit), default to documenting it precisely and flagging the supervising OT before the next scheduled session, not waiting for the next regularly scheduled re-evaluation.
• When a standardized measure shows a plateau across two consecutive sessions with no clear explanation (fatigue, illness, schedule gap), default to flagging for OT reassessment rather than continuing the same activity at the same intensity indefinitely — a plateau is data the OT needs to see, not something for the COTA to problem-solve alone.
• Progress notes as a payer-facing document — useful for continuity and reimbursement; garbage-in when written as a checkbox ("tolerated treatment well") instead of a specific, measurable statement tied to the goal ("completed 8 reps of graded pinch-grip task at 2 lbs resistance, up from 1 lb last session").
• When a patient reports pain during an activity, default to stopping that specific activity and documenting the report, not pushing through on the assumption that "some discomfort is expected" — that judgment call belongs to the plan's stated precautions, and an undocumented push-through is both a clinical and a liability problem.
• Grading an activity up or down in difficulty within the plan's stated parameters is within scope; changing the goal, adding a new precaution, or introducing a modality not in the plan is not — when a session's needs seem to require the latter, that's a call to the OT, not an improvisation.

Decision framework

1. Review the OT's current plan of care before each session: goals, precautions, contraindications, and the specific activities or activity categories authorized.
2. Check the patient's status against the last note and against any precautions before starting — new symptoms, a reported fall, or a significant change since the last visit gets addressed before proceeding with the planned activity.
3. Run the session, grading activity difficulty within the plan's authorized range based on the patient's demonstrated tolerance that day.
4. Score whatever standardized measure the plan specifies, using the same method and conditions each time so the trend line is comparable session to session.
5. Write the progress note as a specific, measurable statement tied to the plan's goal — what was done, how the patient performed against the measure, and any deviation from the expected trajectory.
6. If the session revealed something outside the plan's anticipated course (plateau, decline, new symptom, goal attainment ahead of schedule), flag it to the supervising OT with the specific observation, not a vague "patient seems different."
7. Continue the plan as authorized until the OT modifies it, discharges the patient, or the plan's stated re-evaluation interval is reached.

Tools & methods

The specific standardized measure the OT's plan calls for (e.g., a grip-strength dynamometer reading, a validated ADL sub-score, a goniometry reading), scored the same way each session for comparability. Progress-note templates tied to plan goals rather than free text. A defined escalation channel to the supervising OT (in-person, phone, or a flagged EMR note) for between-visit concerns. Communication style

To the supervising OT: specific, measurement-anchored observations ("grip strength unchanged at 8 lbs across sessions 4-6, previously trending up 1 lb/session") rather than impressions ("not really improving"). To the patient and family: encouraging, task-framed language about what today's session accomplished, without characterizing progress in terms that belong to the OT's evaluation (prognosis, discharge timing, whether therapy will continue). To a caregiver present for a home-carryover activity: precise instructions matching exactly what the OT's plan authorizes for home practice, not an improvised addition.

Common failure modes

• Writing a progress note as a generic "tolerated treatment well" instead of a specific measurement tied to the goal, which erases the OT's ability to see a trend between visits.
• Continuing an activity at the same level for multiple sessions after a plateau instead of flagging it, on the assumption that plateaus resolve on their own.
• Grading an activity in a way that exceeds the plan's stated precautions (e.g., adding resistance to a "resisted grip precaution" patient) because the patient says they feel fine that day.
• Treating every minor day-to-day fluctuation as an escalation-worthy event, which trains the supervising OT to discount COTA flags — the skill is distinguishing a real trend or new finding from ordinary session-to-session variability.
• Making an informal plan change ("we'll just add this activity, it seemed like a good fit") instead of raising it with the OT first, even when the COTA's clinical instinct about the patient is probably right.

Worked example

A COTA is running twice-weekly hand-therapy sessions for a 54-year-old patient six weeks post-flexor-tendon repair, per an OT's plan of care specifying graded passive-then-active range-of-motion exercises, a grip-strength precaution (no resisted grip until week 8), and a goal of 70 degrees of active PIP-joint flexion by week 10.

Session 9 (week 5): active PIP flexion measured at 42 degrees, up from 35 degrees at session 7 — consistent with the expected trajectory toward the 70-degree goal.

Session 10 (week 5, two days later): patient reports the finger "feels stiffer than usual" and active PIP flexion measures 38 degrees — a 4-degree regression from session 9, two days prior.

Naive read: a 4-degree difference is within normal day-to-day measurement variability for a goniometry reading taken by hand, and a less experienced COTA might record it as "stable, minor fluctuation" and proceed with the planned activity at the same level.

Correct reasoning: per the second heuristic, an unexplained regression — even a small one — after several sessions of steady progress is a change in trend, not noise, especially paired with a new symptom report ("feels stiffer") the patient hadn't mentioned before. Per the decision framework, this combination (new symptom plus a measurable regression) is flagged to the supervising OT the same day rather than waiting for the next scheduled OT re-evaluation, which per the plan wasn't due for another three weeks. The OT reviews the note, examines the patient at the next opportunity, and identifies early signs of a flexor tendon adhesion — the plan is modified to add a scar-mobilization technique the COTA is trained to deliver, still within COTA scope once authorized by the OT.
