Casing and cement barriers degrade gradually, and an anomaly like sustained casing pressure is a leading indicator of loss of well control, not an isolated maintenance item to schedule around. Continuing production through an unconfirmed integrity signal trades a visible, short-term production number for an invisible, low-probability, catastrophic tail risk — a trade that looks free until it isn't.
3. Shutting in a well is rarely a neutral, risk-free default even when it looks like the conservative choice. Shut-in/restart cycles can cause formation damage or blockages (wax, hydrate formation in cold conditions) that permanently reduce recoverable reserves — "just shut it in" is a technical decision with a real, sometimes irreversible cost, not a safe no-op.
4. Catastrophic HSE failures in this industry trace back to normalized deviance — a known risk signal repeatedly overridden by production pressure until it compounds. Deepwater Horizon and Piper Alpha are the industry's canonical cases: a single overridden stand-down or ignored anomaly is rarely the actual cause, it's a leading indicator that the organization's risk tolerance has already drifted.
5. Production allocation and regulatory reporting accuracy is a financial and legal control, not administrative overhead. Royalty payments, severance tax, and reserves disclosures all trace back to allocated volumes — an allocation or measurement error doesn't just misstate a number, it compounds into real liability the longer it goes uncorrected.

Mental models & heuristics

• Use decline curve analysis (Arps exponential, hyperbolic, or harmonic models) as the production forecast baseline; treat deviation from the curve, not from a flat historical average, as the real diagnostic signal worth investigating.
• When production is below plan, diagnose in this order: measurement/allocation error first (cheapest to rule out and surprisingly common), then mechanical or artificial-lift issues, then genuine reservoir decline or interference last — reversing this order wastes intervention capital chasing a reservoir explanation for what's actually a meter calibration problem.
• Rank workover and intervention capital by NPV per dollar of spend against each well's remaining decline curve economics, not by which well is generating the most operational attention or complaints.
• Treat any well integrity anomaly — sustained casing pressure, an unexplained wellhead pressure shift — as requiring investigation before continued production, never as something to "monitor for now" while production continues uninterrupted.
• Preserve unconditional stand-down authority for field workers: any worker halts operations on a safety concern without needing manager pre-approval, and a stand-down call that turns out to be unfounded is far cheaper than one that's quietly discouraged and turns out to be real.
• Model produced water handling cost trajectory into long-term field economics, not just current lifting cost — water cut typically rises as a field matures, and a field can become uneconomic from water handling alone well before hydrocarbon volumes justify abandonment on their own.

1. Establish current production against the well or field's decline curve baseline, not a flat historical comparison.
2. Diagnose any shortfall in order: measurement/allocation check, then mechanical/artificial-lift check, then reservoir/interference check.
3. For any well integrity anomaly, suspend normal production assumptions and route to investigation before making any output decision on that well.
4. Rank intervention candidates by NPV per dollar of spend against each well's remaining decline curve economics.
5. Weigh any shut-in/restart decision against formation damage and blockage risk, not solely against current commodity price economics.
6. Confirm HSE stand-down and near-miss reporting are actively being used, not quietly declining under production pressure, as a standing operational health check rather than only a post-incident review.
7. Reconcile allocated and reported volumes against regulatory and royalty obligations on a fixed cadence, independent of whatever production-target pressure is active at the time.

Common failure modes

Chasing a short-term production target by continuing to produce through an unconfirmed well integrity anomaly instead of halting for investigation. Misdiagnosing a measurement or allocation error as a reservoir problem and spending intervention capital on the wrong fix entirely. Allowing stand-down calls to quietly decline under production pressure, letting normalized deviance compound toward a larger incident. Treating a shut-in decision as risk-free because it looks conservative, without accounting for restart formation damage risk. Ignoring the produced water cost trajectory until it forces an unplanned, urgent field abandonment decision instead of a planned one.

Worked example

Field with 40 producing wells, production plan of 12,000 bopd (barrels of oil per day). Actual production has trended to 10,200 bopd over the past three weeks — a 15% shortfall against plan. Executive leadership is pushing to "fix it fast"; a proposal is on the table to accelerate a $2.8M stimulation program across the six lowest-producing wells.

Naive read: approve the stimulation program immediately to visibly address the shortfall on the wells producing the least.

Expert reasoning: before spending on wells, check measurement first. Investigation finds the central tank battery's LACT meter was recalibrated three weeks ago — exactly matching the shortfall's onset — and the recalibration undercounted throughput by roughly 8–9%. Correcting the calibration alone brings measured production from 10,200 bopd to approximately 11,050–11,100 bopd, closing most of the gap from -15% to roughly -8%.
