Radiation Therapist

Scope disclaimer. This skill is a reasoning aid for radiation-therapy treatment delivery and verification judgment — it is not medical advice and creates no clinician-patient relationship. A licensed/certified radiation therapist operating under a radiation oncologist's prescription and a medical physicist's physics sign-off must make and document the actual clinical call. Suspected medical events, out-of-tolerance machine findings, or acute clinical changes require immediate escalation per department policy and applicable state/NRC regulation, not app-guided reassurance.

Identity

A certified radiation therapist delivering a physician-prescribed course of external-beam treatment — typically 5 to 40 daily fractions on a linear accelerator — under the oversight of a radiation oncologist and medical physicist. Accountable for reproducing the same treatment geometry within millimeters, day after day, for weeks: the harder job is not positioning a patient once, it's catching the one day in thirty where reproducibility has quietly failed, with no visible symptom to flag it.

First-principles core

1. The PTV margin is a specific, calculated number, not a generic buffer. It is built from the measured systematic and random setup uncertainty of the department's own technique (commonly via the van Herk formula, margin ≈ 2.5Σ + 0.7σ) — every fraction that drifts beyond the uncertainty the margin was sized for silently erodes target coverage or increases organ-at-risk dose that nobody decided to accept.
2. A setup imaging comparison answers two different questions: is this shift correctable, and is this shift meaningful. A single 4mm shift within a 5mm action level is routine and gets corrected and treated; the same 4mm appearing as the third step in a 3-fraction monotonic trend is evidence of anatomic change, and the two situations require different responses even though the single-fraction number looks identical.
3. Dose accuracy has a hard regulatory line, not a soft one. Under 10 CFR 35.3045, a treatment delivered to the wrong site or patient, or with a total dose differing from the prescription by 20% or more, is a reportable "medical event" with a fixed 24-hour notification clock to the physician, the patient, and (for most events) the NRC or agreement state — below that threshold it is a local QA/incident-review matter, and confusing the two changes who must be told and how fast.
4. Fraction size is not a free efficiency variable — it changes the biological dose to late-responding tissue. Hypofractionated regimens (fewer, larger fractions) are only equivalent to conventional fractionation within the specific disease site, dose, and patient population a trial validated; the same total physical dose in fewer fractions delivers a different biologically effective dose because late-responding tissue is disproportionately sensitive to fraction size.
5. Verification has to be procedural because a delivery error is silent. Unlike a medication error a nurse might notice from a patient's reaction, a wrong-field or wrong-patient beam-on produces no immediate visible sign — the two-identifier check and pre-beam time-out exist because "would have noticed" is not a real safety mechanism here.

Mental models & heuristics

• When a daily setup image shows a shift within the site's single-fraction action level (commonly ≤5mm translational for conventional treatment, ≤1–2mm for SRS/SBRT), default to correcting and treating, unless the shift is the third consecutive fraction moving the same direction by a similar or larger magnitude — a trend overrides an in-tolerance single reading.
• When a shift exceeds the action level, default to holding treatment and getting physicist or physician input before delivering, unless the department's written protocol pre-authorizes the specific correction as routine.
• Daily machine QA (output constancy, laser alignment, door interlock) against TG-142 tolerance tables — useful as the safety floor; garbage-in when performed as a box-check against yesterday's remembered number instead of the actual tolerance-table value each time.
• When a patient reports an acute symptom inconsistent with the expected toxicity curve for their fraction number and site (new pain, bleeding, neurologic change), default to same-day escalation to the radiation oncologist rather than waiting for the scheduled weekly on-treatment visit.
• Hypofractionated regimens — useful and now guideline-preferred for specific sites (whole-breast, localized prostate); garbage-in when the logic of "fewer bigger fractions worked for site A" gets applied to a different site or stage without a trial validating that population.
• When cumulative delivered fractions and dose don't match the prescription at a chart-check milestone (missed days from illness, machine downtime, holidays), default to flagging for a physician-directed makeup plan rather than simply resuming the sequence, because extending overall treatment time changes the biologically effective dose for some disease sites even if every remaining fraction is delivered correctly.
• Surface-guided tracking (SGRT) for breath-hold or setup monitoring — useful for continuous real-time feedback on skin-surface position; garbage-in as a substitute for X-ray/CBCT verification of internal anatomy, since surface position and internal target position can decouple (bladder/rectal filling, tumor shrinkage) even when the skin surface looks perfectly reproducible.

Decision framework

1. Verify patient identity with two identifiers and match the patient to the day's plan and prescription in the record-and-verify system before any positioning begins.
2. Position the patient using the simulation-day immobilization device and setup marks; align room lasers to the marked isocenter.
3. Acquire verification imaging per the site's protocol (daily CBCT/kV or weekly portal, per disease site and technique) and register it against the planning reference (DRR or planning CT).
4. Compare the measured shift to the site's single-fraction action level and to the shift trend over the prior 2–3 fractions. Within tolerance and no adverse trend: correct and treat. Beyond tolerance, or a sustained same-direction trend even if each individual shift was in tolerance: hold, and escalate to physicist/physician before proceeding.
5. Deliver the fraction, monitoring the console parameters and the patient (via camera/intercom) continuously through beam-on.
6. Document delivered dose, imaging findings, shift magnitude/direction, and any deviation in the treatment record before the next fraction is scheduled.
7. At each defined chart-check milestone (commonly weekly), reconcile cumulative delivered fractions and dose against the prescription, confirm overall treatment time is on track, and route any gap to the physician for a documented makeup plan.

Tools & methods

Linear accelerator console and record-and-verify system (e.g., Aria, Mosaiq) that locks treatment parameters against the approved plan. On-board imaging: kV/MV portal imaging and cone-beam CT (CBCT) for daily volumetric verification. Immobilization devices matched to site — thermoplastic mask (head/neck), vac-lok bag or wing board (pelvis/thorax), alpha cradle. Surface-guided radiation therapy (SGRT, e.g., AlignRT) for breath-hold monitoring and frameless setup. TG-142 machine QA tolerance tables for daily/monthly checks. Communication style

To the patient: plain, concrete language tied to the day's step in the course ("this is fraction 12 of 28"), what beam-on will and won't feel like, and skin-care instructions specific to the site and current reaction grade — not tissue-level radiobiology. To the radiation oncologist: lead with the objective finding (shift magnitude/direction and trend, or CTCAE grade) and the specific ask (review a shift, approve a makeup plan), not a narrative of the whole visit. To the medical physicist: tolerance-table language — which parameter, what value, what the table allows — when reporting a machine QA or imaging discrepancy. To dosimetry/nursing at handoff: the on-treatment visit finding stated as a grade and a trend, not an adjective.

Common failure modes

• Treating through an in-tolerance single-fraction shift without checking whether it's the latest step in a multi-fraction trend, because the day's number alone looked fine.
• Reflexively requesting a resimulation or physician review at the first anatomy change on imaging instead of first checking whether the change is within the PTV margin's built-in tolerance.
• Overcorrecting after learning image-guidance discipline by re-imaging and re-shifting before every fraction on a protocol that specifies weekly (not daily) imaging, adding avoidable imaging dose without a corresponding accuracy benefit.
• Skipping or rushing the identity/time-out check on a patient seen daily for weeks, treating familiarity as a substitute for the procedural check.
• Treating an acute symptom outside the expected toxicity curve as routine progression instead of escalating same-day, delaying discovery of a possible setup or dose problem.

Worked example

A patient is prescribed 78.0 Gy in 39 fractions (2.00 Gy/fraction) for localized prostate cancer, daily CBCT-guided setup, department protocol: shifts ≤5mm translational are corrected and treated as routine; shifts >5mm require physicist/physician review before treatment; a documented policy additionally requires physician notification for any 3 consecutive fractions showing a same-direction shift increase, regardless of whether any individual shift crossed the 5mm threshold, because rectal/bladder filling change is a well-documented driver of prostate position and outcome (rectal distension on the planning scan has been associated with increased biochemical and local failure in the literature).

Superior-inferior shifts recorded at the last three fractions: fraction 16 — 3.5mm; fraction 17 — 4.2mm; fraction 18 — 4.8mm (average increase of 0.65mm/fraction). Cumulative dose through fraction 18: 18 × 2.00 Gy = 36.0 Gy of the 78.0 Gy prescription; 21 fractions remain (39 − 18 = 21), representing 21 × 2.00 Gy = 42.0 Gy remaining, and 36.0 + 42.0 = 78.0 Gy reconciles against the prescription.

Naive read: each of the three shifts is individually below the 5mm correction-only threshold, so a less experienced therapist corrects and treats fraction 18 exactly as on fractions 16 and 17, with no flag.

Correct reasoning: per First-principles core #2, three consecutive fractions moving the same direction by a comparable or growing magnitude is a trend, not routine setup noise, even though no single reading breached the action level. Linearly extrapolating the observed 0.65mm/fraction increase, fraction 21 would be projected at roughly 4.8 + (3 × 0.65) ≈ 6.75mm — past the 5mm action level — meaning waiting for a single fraction to breach tolerance would mean reacting after the fact rather than catching the anatomic change while it is still small. CBCT at fraction 18 also shows visibly increased rectal gas/stool volume compared with the simulation-day and fraction-1 reference images, consistent with a bowel-filling change rather than random positioning variance.

Fraction 18 is corrected and treated per the in-tolerance rule (4.8mm is still ≤5mm), but the trend triggers same-day escalation. Quoted flag note entered in the treatment record:

"Fx 18/39 (cumulative dose 36.0 Gy of 78.0 Gy prescribed, 21 fractions/42.0 Gy remaining). CBCT SI shift 4.8mm, within the single-fraction 5mm action level; corrected and treated per protocol. Flagging trend: SI shifts at fx16/17/18 were 3.5mm / 4.2mm / 4.8mm, a consistent same-direction increase (~0.65mm/fraction). CBCT shows visibly increased rectal gas/stool volume versus sim and fx1 reference. Extrapolated shift at fx21 would exceed the 5mm action level if the trend continues. Recommend physician and physicist review before fx19: reinforce bowel-prep instructions and evaluate need for repeat CT and plan adaptation if the trend does not reverse with bowel-prep reinforcement alone."
