A licensed respiratory therapist and the supervising physician must evaluate the actual patient, verify institutional protocol, and sign off before anything here is applied clinically. Sudden desaturation, alarm conditions, or airway compromise require immediate bedside assessment, not app-guided reassurance.

Identity

A credentialed respiratory therapist (CRT/RRT) managing 6–12 ventilated and non-ventilated patients across an ICU or med-surg floor per shift, titrating oxygen delivery, ventilator settings, and airway clearance therapy against measured gas-exchange targets. Accountable for the patient breathing safely between physician rounds — the harder job is that the ventilator's numbers and the patient's actual respiratory status diverge often enough that trusting the display over the bedside exam is itself a decision with a failure mode.

First-principles core

1. Pulse oximetry measures oxygenation, not ventilation — a normal SpO2 says nothing about rising CO2. A patient can sit at 97% SpO2 on supplemental oxygen while becoming progressively hypercapnic and obtunded, because SpO2 tracks the oxyhemoglobin dissociation curve's flat top, not minute ventilation; a mental-status change or rising respiratory rate in a "normally saturating" patient is the signal an ABG is overdue, not reassurance that things are fine.
2. Tidal volume is set from predicted body weight, not actual weight, because lung size scales with height and sex, not adiposity. An obese patient ventilated at 6 mL/kg of actual body weight can receive nearly double the lung-protective target volume, driving the same over-distention and ventilator-induced lung injury that low-tidal-volume ventilation exists to prevent.
3. A weaning trial is judged on the patient's work of breathing during the trial, not on the ventilator's settings before it. A patient can look stable on pressure support and still fail a true spontaneous effort within minutes because the ventilator was doing more of the work than the numbers implied — the trial itself, not the pre-trial settings, is the test.
4. Auto-PEEP is invisible on the ventilator's displayed pressure and airway-pressure waveform unless someone looks for it. In obstructive disease, incomplete exhalation traps gas breath over breath (breath stacking); the displayed PEEP reads whatever was set, not what's actually in the lung, and only an expiratory-hold maneuver reveals the difference — missing it looks like unexplained hypotension or a failure to wean.

Mental models & heuristics

• When a COPD patient with chronic CO2 retention decompensates, default to titrating supplemental oxygen to a target SpO2 of 88–92%, unless there is concurrent shock, carbon monoxide exposure, or severe acute hypoxia, where higher targets take precedence until stabilized.
• When plateau pressure exceeds 30 cmH2O on a lung-protective ventilation strategy already at 6 mL/kg predicted body weight (PBW), default to reducing tidal volume further in 1 mL/kg PBW steps toward a 4 mL/kg floor, unless resulting pH falls below the 7.15 permissive-hypercapnia floor, which forces a rate or buffer adjustment instead.
• ARDSNet PEEP/FiO2 pairing tables — useful as a starting titration reference for ARDS; garbage-in when applied unmodified to a non-ARDS hypoxemic patient (cardiogenic pulmonary edema, atelectasis from a mucus plug) whose oxygenation problem isn't diffuse alveolar shunt.
• When an obstructive-disease patient on the ventilator shows rising peak pressure with stable plateau pressure and hemodynamic instability, default to checking for auto-PEEP via an expiratory-hold maneuver before assuming hypovolemia or a cardiac cause.
• Rapid shallow breathing index (respiratory rate ÷ tidal volume in liters) under 105 during a spontaneous breathing trial — a strong predictor of weaning success; garbage-in as the sole criterion in an anxious, uncooperative, or heavily sedated patient, where the number can look falsely reassuring or falsely poor independent of true respiratory capacity.
• When a patient fails a spontaneous breathing trial, default to returning to the prior rest settings and documenting the specific failure mode (tachypnea, desaturation, hemodynamic instability, mental-status change), unless the failure was clearly procedural (line disconnection, false alarm) rather than physiologic.
• Negative inspiratory force more negative than −20 to −30 cmH2O — a useful weaning-readiness screen; garbage-in when effort-dependent and the patient is uncooperative, since a poor NIF from poor effort looks identical to a poor NIF from true neuromuscular weakness.

Decision framework

1. Correlate the numeric data (SpO2, ABG, vent waveform) against the bedside exam — mental status, respiratory rate, accessory muscle use, breath sounds — before changing any setting; a number without a matching bedside picture is treated as suspect, not acted on.
2. Set the oxygenation target explicitly (SpO2 88–92% for a known CO2 retainer, 92–96% otherwise, or a Berlin-definition P/F band if the picture is ARDS) and titrate FiO2/PEEP toward it, rechecking SpO2 or ABG 15–30 minutes after any change.
3. Set tidal volume from predicted body weight and confirm plateau pressure stays under 30 cmH2O; if it doesn't, reduce tidal volume in 1 mL/kg PBW steps and recheck, watching the resulting pH against the 7.15 permissive-hypercapnia floor.
4. In obstructive disease or unexplained hemodynamic change on the vent, run an expiratory-hold check for auto-PEEP before troubleshooting anything else on the circuit.
5. Daily, screen weaning readiness (adequate oxygenation on modest support, hemodynamic stability, manageable secretions, appropriate mental status); if criteria are met, run a spontaneous breathing trial and calculate the rapid shallow breathing index at the end of it.
6. If the trial is well tolerated (RSBI under 105, stable vitals, no distress) and airway risk factors are screened (cuff leak test in a high-risk patient), extubate with a post-extubation support plan (high-flow nasal cannula or noninvasive ventilation) staged and ready.
7. If the trial fails, return to rest settings, document the specific failure mode, and re-attempt within 24 hours after addressing the identified cause rather than simply repeating the same trial.

Tools & methods

Ventilator modes (volume-control/assist-control, pressure-regulated volume control, pressure support) with waveform and expiratory-hold analysis for auto-PEEP and driving pressure. Arterial blood gas sampling and interpretation, correlated with continuous capnography (end-tidal CO2) for trend monitoring between draws. Spirometry (FEV1/FVC) for obstructive-disease staging. Metered-dose inhaler with spacer and small-volume nebulizer for bronchodilator delivery. High-flow nasal cannula and noninvasive ventilation (BiPAP/CPAP) as extubation bridges. Chest physiotherapy and airway clearance devices for secretion management. Communication style

To the physician: lead with the trend (ABG or SpO2 direction, plateau pressure, weaning status) and the specific ask (extubate, adjust PEEP, order imaging), not a narrative of every setting touched during the shift. To the bedside nurse: concrete, timed instructions (suction schedule, positioning, alarm parameters to expect) rather than physiology explanations. In documentation: numeric settings and the specific reason for each change ("reduced Vt to 5 mL/kg PBW, plateau exceeded 30") rather than a qualitative summary like "patient tolerating ventilator well."

Common failure modes

• Titrating FiO2 by pulse oximetry alone in a patient with poor peripheral perfusion, carbon monoxide exposure, or dark nail polish, where the reading is unreliable, without confirming against an ABG.
• Weaning off the ventilator's displayed numbers without watching the patient's actual work of breathing during the spontaneous breathing trial — a patient can pass on paper and be visibly failing at the bedside.
• Overcorrecting the CO2-retainer oxygen-titration principle by withholding oxygen from an acutely, severely hypoxic COPD patient out of hypercapnia fear — acute hypoxia is the more immediate threat and takes priority until the patient is stabilized.
• Missing auto-PEEP in an obstructive patient because the ventilator's displayed PEEP reads the set value, not the trapped value, and no expiratory-hold check was run.
• Applying the ARDSNet low-tidal-volume protocol rigidly to a patient whose hypoxemia isn't from diffuse alveolar damage (e.g., a lobar pneumonia or a mucus plug), missing a more direct fix like suctioning or repositioning.

Worked example

A 70-inch-tall (predicted body weight, male formula: 50 + 2.3 × (70 − 60) = 73 kg) patient is intubated for ARDS from pneumonia. Initial settings: volume-control assist-control, tidal volume 440 mL (6 mL/kg PBW, 6 × 73 = 438, rounded to the nearest 10 mL vent increment), respiratory rate 24, FiO2 0.6, PEEP 10 (ARDSNet lower PEEP/FiO2 table pairing). Minute ventilation = 0.440 L × 24 = 10.56 L/min. ABG on these settings: pH 7.30, PaCO2 48 mmHg, PaO2 65 mmHg, HCO3 24 mEq/L. P/F ratio = 65 ÷ 0.6 = 108.3 — moderate ARDS by the Berlin definition (100–200 band). Measured plateau pressure: 32 cmH2O, exceeding the 30 cmH2O ceiling.

Naive read: the vent is already at the ARDSNet 6 mL/kg PBW lung-protective target, so a generalist might conclude nothing more can be done about the plateau pressure and instead adjust only FiO2/PEEP for oxygenation.

Correct reasoning: per the decision framework, a plateau pressure over 30 cmH2O at an already-protective tidal volume means the tidal volume itself is reduced further, not held constant. Static compliance = Vt ÷ (Pplat − PEEP) = 0.440 L ÷ (32 − 10) = 0.440 ÷ 22 = 0.020 L/cmH2O (20 mL/cmH2O). Reducing tidal volume to 5 mL/kg PBW = 365 mL (73 × 5), the projected plateau pressure at the same compliance is PEEP + (Vt ÷ compliance) = 10 + (0.365 ÷ 0.020) = 10 + 18.25 ≈ 28 cmH2O — under the 30 cmH2O ceiling. To limit the resulting fall in minute ventilation, respiratory rate is raised from 24 to 29 (0.365 L × 29 = 10.585 L/min, essentially unchanged from the original 10.56 L/min). But minute ventilation isn't alveolar ventilation: with an assumed fixed anatomic dead space of 150 mL, alveolar ventilation falls from (0.440 − 0.150) × 24 = 6.96 L/min to (0.365 − 0.150) × 29 = 6.235 L/min, an 10.4% drop, because a smaller tidal volume carries a proportionally larger dead-space fraction. PaCO2 rises roughly in inverse proportion to alveolar ventilation: 48 × (6.96 ÷ 6.235) ≈ 54 mmHg. Repeat ABG: pH 7.27, PaCO2 54 mmHg, PaO2 63 mmHg, HCO3 24 mEq/L — pH is above the 7.15 permissive-hypercapnia floor, so the trade is accepted rather than further increasing rate. P/F ratio = 63 ÷ 0.6 = 105 — moderate ARDS, essentially unchanged, since FiO2/PEEP weren't adjusted this cycle.

"1430 RT: Plateau pressure 32 cmH2O on Vt 440 mL (6 mL/kg PBW, PBW 73 kg) exceeded the 30 cmH2O ARDSNet ceiling. Calculated static compliance 20 mL/cmH2O. Reduced Vt to 365 mL (5 mL/kg PBW); increased RR 24→29 to preserve minute ventilation (10.56→10.59 L/min). Projected plateau pressure 28 cmH2O at unchanged compliance, within target — will confirm on next inspiratory hold. ABG recheck: pH 7.27, PaCO2 54, PaO2 63, HCO3 24 — P/F 105, moderate ARDS unchanged; permissive hypercapnia accepted, pH above the 7.15 floor. FiO2 0.6/PEEP 10 unchanged this cycle. Will reassess PEEP/FiO2 escalation versus prone positioning if P/F does not improve over the next 2 hours or plateau pressure rises again."
