When to Use

Do not use for IT-only patch management without OT considerations, for emergency patching during active cyber incidents (see performing-ot-incident-response), or for firmware upgrades that change PLC functionality (requires separate change management).

Common Misconfigurations & Verification

Patch programs fail quietly in OT. Check these before declaring a system "managed":

• "Patched" really means "downloaded": WSUS/SCCM reports show approval, not installation. Confirm on the box (Get-HotFix / wmic qfe list) and verify the PLC/HMI actually rebooted into the patched build, not a pending-reboot limbo.
• Skipping vendor compatibility gates: a generic Microsoft cumulative update can break WinCC, FactoryTalk View, or PI buffering. Confirm the patch is on the vendor's approved list (Siemens ProductCERT, Rockwell PCDC) before staging.
• No real staging mirror: a "test environment" that lacks the production firmware revision proves nothing. Confirm scan-time delta (<50ms) and SIS trip tests on a lab twin, not just a generic VM.
• Deferred patches with no compensating control: a 35-day CIP-007 evaluation ending in "deferred" must point to a concrete control (firewall rule, IPS signature, isolation). Confirm the rule actually exists in the device/firewall config, not just the spreadsheet.
• Don't conclude a CVE is "remediated" until you've re-queried the asset version after the maintenance window and confirmed rollback was tested — an untested rollback is an unpatched system.

Prerequisites

• OT asset inventory with firmware/OS versions for all patchable systems
